Student Information Sheet

Complete the Cooperative Education Student Information Sheet

*Your instructor may ask you to provide a Proposal. If so, attach the completed student information sheet to the proposal when submitting.

Student Agreement

Complete the Student Agreement by clicking on the link below

You may need to meet with your instructor as well as your on the job supervisor to verify that the minimum of three objectives are applicable and measureable.

*The agreement must be fully executed before the Cooperative Education Experience can begin.

Student Professional Cooperative Education Agreement

Mid-term Evaluation

A mid-term evaluation needs to be completed by the worksite supervisor and forwarded to you college instructor.

Final Evaluation

A final evaluation needs to be completed by the worksite supervisor and forwarded to your college instructor.

Proposal

Your instructor may ask you to provide a Cooperative Education Experience Proposal. It should contain the following:

  • A statement about the Cooperative Education Experience: when, where, what, how.
  • A statement describing how the Cooperative Education Experience will contribute to your personal/professional development. How will it be beneficial to the business or institution where you are serving the internship
  • A list of (minimal five) objectives-must be product and/or service based
  • A description of the proposed assignments, duties, and activities necessary to complete the Cooperative ducation Experience. They must be aligned with the objectives.
  • Describe how you and/or those on the Cooperative Education Experience site may use your experience and resulting products in the future.
  • Include a cover sheet

Student Information Sheet

Wrap-up Report

Your instructor may ask you for a Narrative Wrap-Up Report describing your over-all experience and lessons learned. It should contain the following:

  • An informational statement about the Cooperative Education Experience: class title, when and where it took place, your stated objectives
  • Touching on each stated objective: did you meet that objective, how so? If not, why not?
  • Did the Cooperative Education Experience meet your expectations? How so or why not?
  • Will the experience contribute to your personal/professional development and goals? How so or why not?
  • Include a cover sheet
